Forms and reports

Every form the well programme accepts, and what each one is for. Select a form to read who files it and when. You start and manage all of them from your applications dashboard.

These forms open a new package. Everything else in this list is added to a package that already exists.

GF · Form 28

Well Construction Permit (Green)

Permission to drill a new well. This is the form that starts a package.

Well driller Available online

What is this form?
What it is
The application for permission to drill a well. It records the property and its owner, what the well is for, how it will be built, and a site plan showing how far the well sits from the septic system, the dwelling and the property lines. The county approves it before any drilling starts.
Who files it
The licensed well driller doing the work. An office clerk or a driller's delegate can fill it in, but a Master Well Driller gives it the final signature.
When it is filed
First, before the well is drilled. Every other form in this list attaches to a package that began here.

WF · Form 30

Well Abandonment Report (White)

A well taken out of service and sealed. Can start its own package.

Well driller Available online

What is this form?
What it is
The report that a well has been sealed and abandoned - what was in the hole, how it was filled and by whom. An abandoned well that is not properly sealed is an open route into the aquifer, which is why this is a filed report and not a note.
Who files it
The well driller who sealed the well.
When it is filed
After the well is sealed. It can start a package of its own when the well being abandoned was never permitted through this portal.

Added to a package you already have

Open the package from your applications dashboard, then add one of these to it. Some become available only after an earlier form has been filed.

YF · Form 29

Well Completion Report (Yellow)

What was actually built, once the well is finished.

Well driller Available online

What is this form?
What it is
The record of the well as constructed, rather than as permitted: the log of what was drilled through, the casing and grout, the yield, and where the well finally went. It is what the state holds as the permanent record of the well.
Who files it
The well driller who drilled it.
When it is filed
After drilling is complete. It stays locked until the 24-Hour Notice of Intent to Drill has been filed on the same package.

BF · Form 31

Unbuilt Well Report (Blue)

A permitted well that was never drilled.

Well driller Available online

What is this form?
What it is
Closes out a permit for a well that in the end was not built, so the permit is not left open indefinitely and the record says what happened.
Who files it
The well driller who held the permit.
When it is filed
Once it is settled that the well will not be drilled.

HF · Form 32

Hydrofracture Report

Fracturing carried out to improve a well's yield.

Well driller Available online

What is this form?
What it is
Records that a completed well was hydrofractured to increase its yield, and what the yield was before and after. Governed by COMAR 26.04.04.28.
Who files it
The well driller who did the work.
When it is filed
After the fracturing, on the package the well was permitted under.

PC · Form 33

Certificate of Potability

A health official certifies the water is safe to drink.

County office Available online

What is this form?
What it is
The local health official's certificate that a sample from the finished well has been analysed by a certified laboratory and the water found potable. It is the document a lender or a buyer usually asks for.
Who files it
A county health-department official. This is not a driller's form, and there is no driller licence check on it.
When it is filed
After the well is completed and a certified laboratory has reported on a sample.

EL · Form 34

Geophysical / Electric Log

A log run down the borehole.

Well driller Available online

What is this form?
What it is
Records that a well was geophysically logged - over what interval, which log types were run - and confirms the copies have been sent to MDE.
Who files it
The well driller.
When it is filed
After the logging, on the existing package.

XR · Form 35

Exception / Variance Request

Asks to be excused from a construction requirement.

Driller and owner Available online

What is this form?
What it is
A request to be let off a requirement for one particular well - most often a setback distance that the site simply cannot meet. It sets out what cannot be met, why, and what is proposed instead.
Who files it
The driller prepares and files it. The property owner signs it last, and their signature is what files it.
When it is filed
After the permit application it relates to has been submitted. It is the next form on the same package - the county cannot decide the application until the variance is in.

SB · Form 36

Setback Measurement Sheet

The measured setback distances on a sheet of their own.

Well driller Available online

What is this form?
What it is
A sheet of the measured distances from the well to each feature COMAR sets a minimum for - the septic system, the dwelling, the property lines and the rest - each shown against the minimum that applies in the county: the State's, or the county's where it requires more. A distance that falls short is recorded as measured, not refused; the exception is asked for on a Variance or Deviation request.
Who files it
The well driller.
When it is filed
Added to a package that already has its permit application. It can be filled in at any time, and submitted once the permit application has been submitted.

DV · Form 37

Well Construction Deviation Application

Asks to build the well differently from the approved permit.

Driller and owner Available online

What is this form?
What it is
A request to construct the well other than as permitted - a different depth, casing or method than the approved application describes. It records what changes and why.
Who files it
The driller prepares and files it. The property owner signs it last, and their signature is what files it.
When it is filed
When it becomes clear the well cannot be built as permitted. Submit the permit application first; the deviation is the next form on the same package.

PY · Form 38

Fee Ledger & Payment Record

What was charged for an application, and what has been paid.

County office Available online

What is this form?
What it is
The office-side ledger for an application: the fee charged and the payments recorded against it, so the money and the permit sit in one place.
Who files it
County office staff. Drillers do not file this.
When it is filed
When a fee is set or a payment is received for an application.

ITD · Form 39

24-Hour Notice of Intent to Drill

Tells the county you are about to drill. Unlocks the completion report.

Well driller Available online

What is this form?
What it is
Notice that drilling is about to begin on a permitted well, so the county has the chance to inspect while the hole is open. Filing it is also what unlocks the Well Completion Report on the same package.
Who files it
The well driller.
When it is filed
At least 24 hours before drilling starts.

TR · Form 40

Permit Transfer Request

Moves a permit to a different driller or owner.

Either side Available online

What is this form?
What it is
A request to transfer a permit to a new driller, a new owner, or both - for instance when the property is sold or another contractor takes over the job. Administrative rather than technical: nothing about the well changes. Governed by COMAR 26.04.04.08.
Who files it
Filed as a request about the permit itself; there is no driller licence check on it.
When it is filed
When the driller or the owner named on a live permit changes.
